DOB OPERATING AND CAPITAL THRESHOLD UPDATE

NYSACRA was informed yesterday that Threshold updates have been approved by the Division of Budget (DOB), effective August 1, 2003.  The most significant change negotiated with DOB is the ability to automatically increase operating thresholds for the annual base trend value.  Other threshold updates of interest are:

·        Program Conversion outliers will now be determined based o­n evaluation of the new rate to the old rate rather than comparison to the Threshold applicable to the proposed program type.  Central Office will perform analysis since the unit reimbursement methodology may differ (e.g., ICF daily fee to IRA monthly fee).

·        The Supervised and Supportive monthly IRA, annual and per diem At-Home Residential Habilitation, and per diem Day Habilitation thresholds have been updated.  As mentioned above, there is now approval to automatically increase the thresholds by the annual base trend value for these services.  A minimum increase of 11 percent was approved to each IRA regional threshold, and a minimum increase of 22 percent was approved for each regional Day Habilitation threshold.

·        Residential and Day Acquisition capital principles and thresholds are now the same for new development and relocations.

·        Respite and other temporary beds may be included in the cost per bed calculation if the residential acquisition, or leased site with a lease term greater than five years, is planned to include such beds.

·        OMR has increased approval authority for residential acquisition proposals, which include funding by other Governmental Entities (e.g., HUD, DHCR, etc.).  Additionally, the Residential Acquisition of Land/or Buildings cost per bed thresholds have been increased by a minimum of 14 percent.

·        Residential Leased Space-Apartment thresholds have been updated.  Instead of separate thresholds for apartments with heat, the thresholds for "without heat" will be used with a Heat Allowance add-on for apartments that include heat.  A minimum of 20 percent increase was approved for each regional "without heat" threshold.

Many of these revisions will result in less "outliers", thereby expediting approvals and, where applicable, provider cash flow.
