Revised Application Announcement |

The Revised Application can be viewed and downloaded to your computer by clicking the following buttons on the OPTS web site; Application for NYS OPTS Pilot Participation (word document), the Budget and Roster for OPTS proposal (Excel format), and the OPTS Service Types (program descriptions). As a reminder, all OPTS proposals must be submitted electronically through the OMRDD Secure Messaging Center.
The Options for People Through Services (OPTS) Revised Application is now available on the NYS OMRDD Web site at www.nysopts.com. Attached for your information is a copy of the revised OPTS Application and budget template.

Highlights of the revision to the OPTS application include:
- Change to Part 1 Identifying Information Section to include request for additional contact information.
- Change to Part II Executive Summary Section with the elimination of questions one - six.
- Change to Part III Narrative Section- with questions one - six being modified to four questions, all but one are yes/no or require a brief description. Additional questions have been reordered or eliminated for a total of sixteen questions.
- Change to Part IV Evaluation Section with examples of Personal Outcomes (required) and Organizational Outcomes (optional) are given. The submission of a consumer satisfaction survey is no longer required with the initial application, but may be submitted during the technical assistance phase. Additional evaluation guidance documents are provided as attachments.
- Modifications to the Part V Budget and Roster have been made clearer regarding required information. A separate Consumer Verification page has been added and requires besides a contact name and phone number, the best time of day to call and an alternate phone number.
- New Information Attachments include: Budget Unit Discussion Points, Capital Finance Services Property Questionnaire, and an OPTS Evaluation Guidelines which include examples of acceptable evaluation approaches.
Please note that the older version of the OPTS application can be used until November 1, 2006 at which time the Revised Application and budget template will be required.
