Title: Mount Vernon residential reconfiguration and relocation project.

EXECUTIVE SUMMARY


Westchester Jewish Community Services intends through this NYS-OPTS proposal to reconfigure and relocate its two CR supportive apartments in Mt. Vernon to a supervised IRA also located in Mt. Vernon. Six individuals will be served in this new program. Two individuals are current residents and one person has requested a transfer from another WJCS IRA to this new setting. The other three adults have signed MOU’s and are registered on the Westchester County DCMH registry and approved for services by OMRDD. WJCS intends to utilize HUD 811 funding for a share of the capital expenses for this project.

WJCS has met with all the consumers and families to discuss this proposed program change and to discuss how this would be in the best interest of the consumers. . Each person has met their future ‘housemate.’ WJCS considers this to be the beginning phase of a longer-term commitment and process to develop this home. Once the residence is identified, WJCS will facilitate monthly educational/support/problem-solving meetings to discuss the services and opportunities within this residence. Through these ongoing meetings, other needs will be articulated and incorporated into the program design for the residence. This process was utilized in our most recent residential development. The consumers, families and the agency determined it to be highly beneficial process because relationships were built and everyone learned a great deal about the agency and the services that they would receive. Very importantly, they had extensive input into the physical plant and program design of the residence.

WJCS will commit staff from the organization to assist with this development. The Assistant Executive Director, Steven Yellen will lead this development process. The facilities director will be highly involved in the HUD 811 process and the renovations. Several supervisory and management staff (e.g. Directors of residential and clinical services, nursing coordinator, MSC supervisor, and coordinator of training and QA) will be included in the consumer and parent program development process.

WJCS will utilize several consumer satisfaction surveys to measure the effectiveness of this program. The development phase will be evaluated just prior to licensure. A post-admission satisfaction survey will be conducted followed by an annual survey. In addition, quarterly meetings will be arranged individually and collectively for the consumers and parents to discuss and document their respective levels and satisfaction with individualized services.
